Physician & Physician Assistant: the Idaho Path

Idaho has no public medical school of its own. It buys seats: 40 each year at the University of Washington School of Medicine through the WWAMI program, with the first 18 months in Moscow. A private osteopathic school, ICOM in Meridian, trains more. Physician pay clears every income bar by a wide margin, but the road runs 11 years or more past high school. The physician assistant reaches a family income in about half that time.

Why this field is on the list · as of September 2026

$347,160Median family medicine physician pay, state of Idaho, May 2025
$133,790Median physician assistant pay, state of Idaho, May 2025
$104,611Idaho living wage, one earner, three children

It passes the one-income test easily, once the training is done. Idaho’s median family physician earns more than three times the three-child bar, and internal medicine ($355,390) and pediatrics ($297,680) are close behind. Even the 25th percentile of family physicians ($210,650) is twice the bar. A physician assistant at the median clears it by nearly $30,000, and in Coeur d’Alene the PA median is $135,240.

The catch is time and debt. A physician needs four years of college, four of medical school and three or more of residency. ICOM’s estimated first-year cost of attendance for 2026–27 is $110,825, with tuition alone at $70,090. WWAMI students pay much less, but they owe Idaho four years of full-time practice here in return, or they repay the difference. A physician assistant needs a bachelor’s degree and a two-year master’s, and is working by about 24.

The work stays in human hands. AI already drafts chart notes and suggests diagnoses, and it will do more of both. The physician in Bonners Ferry still examines the child, weighs what the parents did not say, and signs the order. In a small town that doctor is also the one the family will see at church on Sunday. Judgment and accountability do not transfer to software.

Watch for: north Idaho is short of primary-care doctors. The only family medicine residency in north Idaho is in Coeur d’Alene, and most of its graduates stay in the panhandle. A student who trains here is likely to practice here.

01 · The ladderFrom home-school science to a practice

The physician and PA roads share their first rungs. A student can climb the early ones while still living at home and decide later which road fits.

  1. Patient-care work at 16 to 18

Medical schools and PA programs want to see that a student has worked around patients. A CNA course at 16 (see nursing), an EMT license (see EMT), or medical-assistant or scribe work all count, and all pay while the student studies.

Medical assistant median $44,700 · a paying job during college, not a family income

  2. A bachelor’s degree with the science prerequisites

Biology, general and organic chemistry, physics, biochemistry, anatomy and physiology, and statistics. NIC’s pre-professional transfer courses cover the first two years close to home, and dual credit can start them in high school. The University of Idaho (Moscow) and Idaho State University are the in-state places to finish.

  3. Medical school: WWAMI or ICOM

University of Idaho WWAMI. 40 Idaho residents are admitted each year (160 across four years). The first 18 months of classes are in Moscow, and clinical training follows across Idaho and the five-state WWAMI region. The degree is the University of Washington MD. Students who pay WWAMI tuition rates owe four years of full-time practice in Idaho. A separate track called TRUST prepares students for rural and underserved practice.

Idaho College of Osteopathic Medicine (ICOM), Meridian. A private, for-profit school founded in 2016, with about 600 students in total. Its entering class is more than twice the size of WWAMI’s, and its graduate earns the DO degree. The cost is high.

  4. Residency

Three years for family medicine, longer for surgery and other specialties. The Kootenai Clinic Family Medicine Residency in Coeur d’Alene is part of the UW Family Medicine Residency Network, the only family medicine residency in north Idaho, with rural rotations in Sandpoint.

Residents are paid a salary while training, well below physician pay.

  5. Physician in practice

    Family medicine median $347,160 · internal medicine $355,390 · pediatrics $297,680 · OB/GYN $223,560 · emergency medicine $221,430 · all far above the bar

02 · BranchesThe shorter road, and the choices along the long one

Physician assistant

Idaho State University’s Master of Physician Assistant Studies runs 24 months (12 of classes and 12 of clinical rotations), at Pocatello and Meridian and at a partner site at the College of Idaho. It takes a bachelor’s degree, a 3.0 prerequisite GPA, the GRE, and courses in biochemistry, microbiology, anatomy, physiology, statistics and psychology. Applications go through CASPA by November 1. ISU does not require patient-care hours, but they make any application stronger.

Pay: Idaho median $133,790; Coeur d’Alene $135,240. Clears the three-child bar.

Family medicine in a small town

The road most needed in the panhandle. Family physicians deliver babies, set bones, treat the elderly and cover the emergency room at small hospitals like Bonner General and Boundary Community Hospital.

Schedule: Clinic days plus a share of call. In a small town the call can be heavy; ask how many doctors share it.

Other doctoral fields

Dentistry, pharmacy, optometry, podiatry and chiropractic are also doctoral paths. Idaho medians: optometrist $103,390, podiatrist $124,610, chiropractor $80,770. See dentistry and pharmacy.

Why it matters: several of these reach a family income with less debt and fewer years than an MD.

03 · For the familyThe schedule, for a father and for a mother

04 · The home-school yearsWhat to add to the core path

Grades 9–10

  • Everything in the shared core, with algebra through precalculus on schedule
  • Full-year biology and chemistry with labs
  • Volunteer hours at a clinic, hospital, hospice or nursing home

Grades 11–12

  • A CNA course at 16, or an EMT license with a parent’s consent
  • NIC dual credit in chemistry, biology, English and statistics, paid largely through Advanced Opportunities
  • Shadow a family physician and a physician assistant, and ask each about their training

After high school

  • Two years of transfer courses at NIC, then a bachelor’s at the University of Idaho or ISU
  • Keep a paid patient-care job throughout
  • Apply to WWAMI and ICOM, or to PA programs through CASPA

Tie to the Verification Track

Medicine runs on checking. A lab value that makes no sense gets drawn again before anyone acts on it. An AI suggestion gets weighed against the patient in the room. The Verification Track trains that same habit of testing a claim against the evidence before trusting it.
